Important information and forms for the new school year

FCPS Required Forms for All Students

Emergency Care Form	Parents can update in SIS ParentVUE or fill out the hard copy going home in students' Thursday folders If updating online, please return the hard copy marked "updated online"
Health Information	Complete and return the hard copy going home in students' Thursday folders OR complete in SIS Parent VUE Click on DOCUMENTS tab This will walk you through how to update the "Health Information Form" which is an annual first week of school requirement
Student Rights & Responsibilities	Students will review the Student's Rights and Responsibilities (SR&R) in the second week of school. The SR&R Booklet can also be found on the FCPS website. Parents should sign the SR&R form in SIS ParentVue.
Parent Consent for Digital Tools	Digital Resources - Parent Consent - Review FCPS approved digital resources that require your approval Parental Digital Consent (Sign in with SIS ParentVUE credentials)
